3.  Diabetes and related remedies in medieval Persian medicine 
Diabetes Mellitus is a common metabolic disorder presenting increased amounts of serum glucose and will cover 5.4% of population by year 2025. Accordingly, this review was performed to gather and discuss the stand points on diagnosis, pathophysiology, non-pharmacological therapy and drug management of diabetes this disorder as described in medieval Persian medicine. To this, reports on diabetes were collected and analyzed from selected medical and pharmaceutical textbooks of Traditional Persian Medicine. A search on databases as Pubmed, Sciencedirect, Scopus and Google scholar was also performed to reconfirm the Anti diabetic activities of reported herbs. The term, Ziabites, was used to describe what is now spoken as diabetes. It was reported that Ziabites, is highly associated with kidney function. Etiologically, Ziabites was characterized as kidney hot or cold dystemperament as well as diffusion of fluid from other organs such as liver and intestines into the kidneys. This disorder was categorized into main types as hot (Ziabites-e-har) and cold (Ziabites-e-barid) as well as sweet urine (Bole-e-shirin). Most medieval cite signs of Ziabites were remarked as unusual and excessive thirst, frequent urination and polydipsia. On the management, life style modification and observing the essential rules of prevention in Persian medicine as well as herbal therapy and special simple manipulations were recommended. Current investigation was done to clarify the knowledge of medieval scientists on diabetes and related interventions. Reported remedies which are based on centuries of experience might be of beneficial for- further studies to the management of diabetes.

4.  Analysis of the essential oil components from different Carum copticum L. samples from Iran

Background:
The family Apiaceae is defined with the diversity of essential oil. Fruits of Ajwain (Carum copticum), a famous herb of Apiaceae, accumulate up to 5% essential oil which is remarked as important natural product for food and flavoring industry, as well as pharmacological approaches. It is believed that differences in essential oil profile in a certain plant are resulted from various cultivation situations and locations, time of cultivation and also different extracting method.
Objective:
Present study aimed to evaluate major components of ten different collected Ajwain samples from random cultivation locations of Iran.
Materials and Methods:
Samples were individually subjected to hydrodistillation using a Clevenger-type apparatus for the extraction of essential oil. GC/MS analysis for samples was carried out using Agilent technologies model 7890A gas chromatograph with a mass detector.
Results:
The yield of extracted essential oil was calculated as 2.2 to 4.8% (v/w) for ten samples. Major oil components were thymol, para-cymene and gamma-terpinene. Five of ten samples have thymol as the main component with amount of 35.04 to 63.31%. On the other hand, for four samples, para-cymene was major with amount of 40.20 to 57.31% and one sample had gamma-terpinene as main constituent containing 37.43% of total oil. Accordingly, three different chemotypes, thymol, para-cymene and gamma-terpinene can be speculated from collected samples.
Conclusion:
While these components possess pharmacological effect, screening of different chemotypes not only represent the effect of cultivation situations and locations but also can be beneficial in further investigation.

5.  Geriatric management in medieval Persian medicine

In Iran, a large group of patients are elderly people and they intend to have natural remedies as treatment. These remedies are rooted in historical of Persian and humoral medicine with a backbone of more than 1000 years. The current study was conducted to draw together medieval pharmacological information related to geriatric medicine from some of the most often manuscripts of traditional Persian medicine. Moreover, we investigated the efficacy of medicinal plants through a search of the PubMed, Scopus and Google Scholar databases. In the medieval Persian documents, digestible and a small amount of food such as chicken broth, honey, fig and plum at frequent intervals as well as body massage and morning unctioning are highly recommended. In the field of pharmacotherapy, 35 herbs related to 25 families were identified. Plants were classified as tonic, anti-aging, appetizer, memory and mood enhancer, topical analgesic and laxative as well as health improvement agents. Other than historical elucidation, this paper presents medical and pharmacological approaches that medieval Persian practitioners applied to deal with geriatric complications.

6.  Sphygmology of Ibn Sina, a Message for Future 
Sphygmology is a 2000-year-old method that is still used in some traditional medicine systems, mainly Iranian, Chinese and Ayurveda. Ibn Sina is one of the most popular physicians in the history of Sphygmology, who had made great contributions to this method, but few meticulous studies have been made on his concepts. In this article, his standpoints on Pulsology are studied. Ibn Sina has mentioned 10 parameters for pulse palpation, and 13 compound pulses, as well. Besides, some pulse patterns applicable for differentiating similar diseases have been introduced. Nowadays some concepts on pulse patterns, such as pulse change in sleep and pregnancy have been studied; but unfortunately many of the ideas on Pulsology have not yet been examined. Sphygmology, as an accessible and inexpensive means, having a substantial diagnosis yield, has been accepted by the people since its rise, and its development over centuries ago. Although some new instruments have been innovated, which can measure different parameters of pulse, yet sphygmology can be one of the most commonly usable diagnostic methods in future, not only applicable by cardiologists but also by all the physicians.

7.  Efficacy of Nepeta Menthoides Boiss and Buhse Freeze-Dried Aqueous Extract on Anxiety of Patients with Depression: A Double-Blind Randomized Controlled Clinical Trial 
Background:
The efficacy and safety of Nepeta menthoides freeze-dried aqueous extract were assessed on the anxiety of patients suffering from depression.
Method:
Patients received either N. menthoides formulation (400 mg/BID) or Sertraline (50 mg/BID) for 4 weeks. The Beck Anxiety Inventory (BAI) scales were used to assess the anxiety in two-week intervals (2nd, 4th, and 6th weeks). Folin-Ciocalteu and Dowd methods were used to determine the formulation of total phenol and flavonoid contents.
Results:
Compared with Sertraline, N. menthoides showed a higher reduction in BAI scores in the 2nd (16.52±8.07 vs. 21.38±10.98, P<0.05) and 4th week (11.55±6.74 vs. 20.47±11.53, P<0.05) along with a reduction in the recurrence rate and side effects. Total phenolic and flavonoid contents revealed in the presence of 127.09±0.43 mg GAE/g and 16.93±0.09 mg Q/g of extract.
Conclusion:
N. menthoides could be effective in the control and introducing a delay in recurrence of anxiety in patients with depression.

8.  Non-Pharmacological Treatments of Allergic Rhinitis (Neglected Treatments) 
Background:
Allergic rhinitis is the most common diseases affecting people in industrialized society. However, this is not a new disease and it was clinically described and treated for the first time by Rhazes (865-925 CE). The disease was also mentioned in “The Canon of Medicine” by Avicenna (980–1037).
Methods:
We searched in Scopus, Web of Science, and PubMed for “allergic rhinitis”, “interactions”, “non-prescription”, “prescription”, and in electronic copies of ITM sources the “canon” and “Al-Havi”.
Results:
Both Persian pioneers of Medicine recommended non-pharmacologic management as an important phase of the therapy. Their recommendations consisted of avoiding overeating and polydipsia, massage of the lower extremities, adjusting the duration and time of sleep, sleeping in the supine position, avoiding exposure of the head to cold air and taking a shower early in the morning.
Conclusion:
Although some aspects of their recommendations, such as massage of the lower extremities, avoiding of overeating and adjusting of sleep pattern were approved, but further cross-sectional and prospective studies are needed to confirm other non-pharmacological treatments.

11.  Flixweed vs. Polyethylene Glycol in the Treatment of Childhood Functional Constipation: A Randomized Clinical Trial

Background:
Polyethylene glycol (PEG) is often considered as the first-line treatment for functional constipation in children. Descurainia sophia (L.) Webb et Berth (D. sophia) is a safe recommended medicine in Iranian folk and Traditional Persian Medicine for the treatment of constipation.
Objectives:
To clinically compare D. sophia with PEG 4000 (without electrolyte) in pediatric constipation and to assess its efficacy and side effects.
Patients and Methods:
120 patients aged 2 - 12 years with constipation for at least 3 months were included in an 8 weeks lasting randomized controlled trial within two parallel-groups. Children received either PEG, 0.4 g/kg/day, or D. sophia seeds, 2 grams (for children aged 2 - 4 years) and 3 grams (for those aged > 4 years) per day.
Results:
A total of 109 patients completed the study (56 in D. sophia and 53 in PEG group). At the end of the study, 36 (64.3%) patients in D. sophia group and 29 (54.7%) in PEG group were out of Rome III criteria (P = 0.205). Median weekly stool frequency in 0, 1, 2, 3 weeks of the treatment was found to be 2, 5, 5, 5 in D. sophia and 3, 4, 4, 5 in PEG group (P = 0.139, 0.076, 0.844, 0.294), respectively. The number of patients who suffered flatulence was less (5, 8.9%) in D. sophia group as compared to PEG group (6, 11.3%) at the end of the trial (P = 0.461). D. sophia taste was less tolerated.
Conclusions:
D. sophia is introduced as a cheap and available medication which can be applied as a safe alternative to conventional PEG in the management of pediatric chronic functional constipation.

12.  Types of headache and those remedies in traditional persian medicine

The history of headache, as a common neurological complication, goes back to almost 9000 years ago. Many ancient civilizations present references to headaches and the coherent treatment strategies. Accordingly, several documents comprising headache complications embodying precise medical information stem from Traditional Persian Medicine (TPM) that can provide useful opportunities for more comprehensive treatment. We conducted a survey on headache through original important pharmacopeias and other important medical manuscripts of TPM which were written during 9th to 19th centuries and have derived all headache categories and herbal remedies. An extensive search of scientific data banks, such as Medline and Scopus, has also been exercised to find results relating to the anti-inflammatory, anti-nociceptive, and analgesic effects of denoted medicinal herbs. The concept of headache and treatments in TPM covers over 20 various types of headache and more than 160 different medicinal plants administered for oral, topical, and nasal application according to 1000 years of the subject documents. Nearly, 60% of remarked medicinal herbs have related anti-inflammatory or analgesic effects and some current headache types have similarities and conformities to those of traditional types. Beside historical approaches, there are many possible and available strategies that can lead to development of new and effective headache treatment from medicinal plants so that this study can provide beneficial information on clinical remedies based on centuries of experience in the field of headache which can stand as a new candidate for further investigations.
